Summary |
Coordinates the functional assessment of the client’s abilities and development of an Individual Habilitation Plan, monitors client progress, and chairs the Individual Habilitation Plan Meetings.
Principal Duties/Responsibilities: • Prepares an Individual Habilitation Plan for annual and semi-annual staffing.
• Completes individual client assessment, client preference, recreation assessment and other necessary/required assessment annually.
• Collects, and analyzes client behavior data monthly.
• Collects, and analyzes client performance data monthly.
• Writes a monthly progress note on each client.
• Develops, plans, and monitors CILA activities for residents
• Works with CILA Director, Team Leaders, day program staff, and other community living staff.
• Informs the CILA Director/Team Leader of errors in staff data collection or reporting.
• Informs the CILA Director, and Team Leader in writing, when program services have not been followed.
• Maintains an awareness of needs of each resident and informs the Team Leader and CILA Director in writing.
• *Submits Bi-weekly QMRP reports to the CILA Director.
• Develops specific, structured, learning and maintenance programs directed toward independence in the self-administration of prescribed medication for each resident.
• Participates in CILA licensure inspections with the Illinois Department of Mental Health and Developmental Disabilities.
• Participates in the surveys and inspections conducted by applicable regulatory and lending bodies.
• Serves as a member of the Behavior Management Committee.
• Performs the specific Qualified Mental Retardation Professional Responsibilities (QMRP) attached hereto.

| Education/Experience |
• The position requires a bachelor’s degree from an accredited college or university with a major in one of the behavioral sciences, and at least one year of experience with the Developmentally Disabled population.
• This position requires a valid driver’s license and a motor vehicle report acceptable to the agency’s insurance broker.
• Demonstrated the experiences, beliefs, attitude and awareness that indicate cultural sensitivity to the client population served.
• Ability to partner with clients individually, and as a family, to help them mobilize resources and their inner capacity to address problems they faces.
• Ability to coordinate effectively with other team members, other Agency resources and outside services in addressing client needs.
